We understand the significant impact pain and injuries can have on your life and aim to make your journey with us as smooth as possible while providing the best long term outcome possible.
There's no single "right" path through Formotion, most clients move through these stages, but you can add, swap or step on or off at any point along the way.
Start Here
1. Book an appointment
Get started by booking online, or reach out to our team directly to find a time that suits you.
2. Your initial consult
You'll see one of our Physiotherapists or Exercise Physiologist for your first session, most clients start with Physio if you have an injury that needs a diagnosis first.
If you want to skip straight to exercise based rehab you can book with either.
3. Continue with 1:1 appointments
Work through your treatment plan or exercise program with your Physio or Exercise Physiologist during individual sessions and by following the app at home or in a gym. Your clinician will guide you on how many sessions you are likely to need.
Optional: Add on a Formotion Open Membership if you would like to work through it from our Osborne Park space.
4. Add focus classes
Layer group focus classes or semi-private sessions alongside your 1:1s, or move into them later, whenever it suits you.
Focus classes work well when done alongside our memberships if you feel you need a little more attention on your technique without booking a 1:1 session.

5. Book a Performance Test
Optional - Once you have moved beyond pain or injury (or close to it) or are training to improve your performance in your sport or life.
This is a good option for athletes or those who like to have highly objective data to guide your exercise program.

Step on or off anytime
This isn't a one-way path. At any stage, you are free to move back to 1:1 appointments, add in focus classes, change memberships or just take a break to try something else!
Before your first appointment

You don’t need a referral to see us privately using your private health card.
You will need to get a referral from your GP if you would like to claim through medicare for chronic conditions or DVA. Please include this information in your client form or email it to us prior to your appointment.
For worker’s compensation or motor vehicle accident claims you will need to have a claim number and ideally a GP referral, or be in the process of getting one. -
If you book online you will fill in the form as part of the booking process. If not this will be emailed to you before your appointment and can be filled in online.
We ask that you do is as soon as possible to allow us to prepare for your appointment.
Please let us know if you don’t receive your new client form. Sometimes they do end up in junk mail or under the “promotions” section if you are a Gmail user.
Note: if you are already a client of ours you won’t need to fill this in, however we may send you a separate form if it has been a while since you saw us to update your details. -
On the day of your appointment, we recommend wearing training clothes, or something you can move in easily which allows us to see the affected area.
We also ask that you bring the following to your appointment:
Your private health card if you are paying privately, we accept digital and physical. All of our private fees can be viewed here.
Any scans, doctors letters or other relevant documentation if you haven’t already included these in your new client form
Most of our Physiotherapy treatment plans and programs are delivered via Physiapp with videos and instructions, if you have a chance to download this before your appointment this will help streamline the process.

We are a bit different to traditional Physio clinics you may have been to in that we are based inside open spacious gyms. If you are not sure if you are in the right space, give us a call! Our admin team is remote so when you arrive just have a seat and your Physio /exercise Physiologist will come and get you when it is time for your appointment.
Make sure to check the location of your appointment on your confirmation email and reminder text to make sure you come to the right one!
We recommend you use Google Maps rather than Apple for best directions!
OSBORNE PARK:
5/34 Sundercombe St, Osborne Park 6017Parking: There is free parking in front of our building. Enter through the glass front door and have a seat in the waiting area where your Physio will come and find you here when it is time for your appointment.
The bathrooms are located to the right of the front door, or at the back if required.
JOONDALUP:
2/320 Joondalup Dr, Joondalup 6027(Inside Mobius Health and Performance)
Parking: Park across the road in the Harvey Norman parking lot for easiest access to our rooms.
Enter through the glass sliding door, turn right and have a seat in our waiting area where your Physio will come and find you when it is time for your appointment.
If you have mobility issues and are worried about crossing the road then you can also park on the other side of the gym in front of 99 Bikes, just ask someone to point you in the right direction of our rooms once you have entered.The bathrooms are at the back of the gym.

At your first appointment you can expect us to conduct a thorough assessment where we will ask you questions about yourself, your health, your injury and your goals.
This will be followed by a physical examination where we will conduct tests to look at your injury and how you move.
Treatment may come in the form of massage or hands on therapy, exercise, movement advice and modification, dry needling, taping or anything else we think is the best path for you clinically.
Our treatment is guided by both evidence-based practice and your individual preferences and goals.Our treatment and exercise plans are generally delivered within the first or second session through Physiapp so you won’t leave trying to decipher stick figure drawings or trying to remember what was said!

Our team and your Physio will work hard behind the scenes to ensure that you can make the most progress possible between your appointments.
You will receive an exercise program via Physiapp if applicable, and a personalised treatment plan to map out your individual patient journey. Make sure to check your emails so you can access your exercise program and treatment plan!
We will also communicate and liaise with your coach/GP/specialist as required.
You can also contact your Physio with any injury / program related questions either via Physiapp or their email.
