Worker’s Compensation
Formotion Physio has experience in helping you navigate through WorkCover / Workers Compensation claims at either of our clinics in Osborne Park or Joondalup.
Our approach focuses on delivering holistic treatment plans that consider a bio-psychosocial model that helps you to recover quickly while ensuring a smooth and stress free administrative process to help you return to work as quickly and safely as possible.
Key Points:
Physiotherapy and exercise physiology may be provided to workers who have sustained a workplace injury, this is funded through the workplace’s insurance cover.
You’ll need a GP referral, claim acceptance letter and claim number.
Appointments for your specific work related injury are fully covered by the insurer.
You can book online by selecting a “Workers Compensation / Motor Vehicle Accident - New Assessment”
or contact the clinic to make a booking.
How do I get started?
If you have sustained a workplace injury you will first need to talk to your employer and adhere to the Worker’s Compensation guidelines of your work place.
Once you have done this you will generally need a referral and a first certificate of capacity from your GP.
You will need the following details in order to start your physiotherapy treatment under Worker’s Compensation:
Claim acceptance letter from your relevant insuring company from work.
Claim details – including claim number, case manager name and contact details.
If you do not have this information you can still pay for your appointments privately. In some cases you may be able to claim these appointments back later - but make sure you check this first!
Who is responsible for bills?
Formotion will liaise with your case manager from the relevant insurance company to seek approval of physiotherapy treatment plans under the WorkCoverWA rates.
Once approval has been granted, all fees will be directed to the insuring company with no out of pocket expense for clients. However, any treatment outside of the proposed plan will be your responsibility.
Do I need to stay with the Physiotherapist I am referred to?
No. If you would like to change providers for whatever reason you should talk to you case manager to discuss this. If you have a provider you would like to you use you can discuss this with them prior and they may be able to help you with the changeover process as well as obtain any relevant handovers from the previous Physiotherapist.

Cancellation Policy
It is important to note that missed appointments and late cancellations fees will apply as per our cancellation policy. These are not covered by WorkCover or your insurance company and will need to be paid for privately if you miss an appointment or cancel late.

Motor Vehicle Accident Rehabilitation (MVA)
We can provide Physiotherapy or Exercise Physiology rehabilitation for injuries sustained in car or motorbike accidents that have been accepted for coverage by ICWA (Insurance Commission of WA).
Common injuries we see are whiplash, concussions, knee/hip injuries and other impact or high speed related injuries caused by traffic accidents.
We will work with your insurer, GP and any other health professionals to help you get back to on track with as little stress as possible.
Key Points:
You will need a claim number and confirmation of acceptance from ICWA before claiming your sessions with us.
You can book your sessions online by selecting the “Worker’s Compensation / MVA” Initial Assessment.
It is recommended to also get a GP referral or have a GP review so that we are able to liaise with them where necessary.
If you do not have this you are still able to see us for Physiotherapy however you will need to pay for your sessions upfront, you may be able to claim these later if your claim is then accepted.

”If you have been injured in a motor vehicle crash, you can make a claim to have approved treatment paid by us. We encourage you to seek treatment from your doctor or a hospital as soon as possible to assist your recovery.
During the claims process, the following occurs:
You report the crash and make a claim.
We send you a confirmation via SMS, email or post and your unique treatment number.
We send you an SMS with a link to access your digital claiming card.
You start using your treatment number, and digital claiming card (if downloaded) to have treatments paid.
A claims management officer is assigned to your claim and assesses the information you have provided.
If we need more information about the crash, we may contact you to ask for more details.
We determine liability for the crash, and if another driver was at fault, the claims officer will approve treatment and let you know what is covered.
Throughout your recovery, we will touch base with you to check on your progress and see if extra treatment is required.
When you have recovered, we will call you to discuss finalising your claim, and confirm in writing.
The claims process duration can vary between people. This is because everyone's experience is different, so every claim is different.”
